Advice on Operational Incidents 

The procedure for reporting fail to calls and trains stopped short in a platform, have been under discussion at Company Council level for some time, we are pleased to say that changes have been agreed, the correct reporting procedure for both of these incidents, are as follows.

Failure to call
It has long been our position that a Driver could not report a fail to call if the Driver was unaware that s/he has had a fail to call, by definition it is something that a Driver was unaware that s/he should do, how then could they report it if they were unaware that it had happened?

We are pleased to say that we have now agreed with South West Trains that the current system is open to misunderstanding, so to clarify the situation, the reporting procedures will also change from 01 May 2010 

The new arrangement is that a Driver must report a fail to call as soon as s/he becomes aware that they have had such an incident, put simply, if a passenger complains to you that you were meant to have stopped at the previous station         (and you were), you should report it there and then, or if the Guard tells you via the cab to cab that you were meant to stop at the last station, then report it as soon as it is safe to do so, either via the CSR/NRN or at the next station.

Stopped Short in a Platform

If you are responsible for stopping the train in a platform and the guard informs you that you have stopped short, and the doors have not been released, you may now move the train (with the agreement of the Guard), up to the correct stop, 

and then report what has happened prior to leaving the station.

If your GSM-R or NRN and the Signal Post Telephone are not working you should ask the Guard to report the incident via their mobile phone before you leave the station.
If no form of communication is available between you and the Signaller, then you may continue with your journey, but you must report the incident at the first available opportunity.

If you have stopped short because of a TPWS activation or any other safety system activation, or you are unaware why the train has stopped,

You must report the incident to the Signalman, prior to moving the train.

It is important to stress that the reporting procedures have not changed for stations which have permissive working.

At these locations you must still speak to the Signalman before moving your train, once permission to move has been
obtained, you should liaise with your Guard prior to moving your train

These changes to the reporting procedures only apply to a stop short initiated by the Driver, 

If you are in any doubt, then report the incident prior to moving the train
